Billing and Patient Financial Responsibility
As a patient, you and your insurance company have an agreed financial responsibility as a part of your insurance plan.
When you become a patient of PCMD, you sign a patient financial responsibility agreement.
You agree to be responsible for your portion of fees not covered by your insurance company.
Your co-payment and co-insurance are required to be paid prior to receiving services.
If you have a deductible insurance plan and have not met your deductible, our fee for service is expected to be paid at the office visit.
We cannot bill you for same day professional service.
You will only receive a bill for services not covered by your insurance plan.
