We provide primary care, annual exams and management of chronic diseases.

We treat hypertension, diabetes, elevated cholesterol and weight loss with semaglutide.

Women’s health is now available with our Nurse Practitioner.

When you become a patient of
Personal Concierge MD, you sign a patient financial responsibility agreement.

You agree to be responsible for your portion of fees not covered by your insurance company.

Your co-payment and co-insurance are required to be paid prior to receiving services.

If you have a deductible insurance plan and have not met your deductible, our fee for service is expected to be paid at the office visit.

We cannot bill you for same day professional service.

You will only receive a bill for services not covered by your insurance plan.

If you have questions about your insurance plan, please call your insurance company.

Concierge Upgrade

If you want more personal care , we do offer “one on one” concierge services with our physician.

For $167 per month, receive the benefits of 24/7 access to the physician and same day or next day appointments.

Private insurance is required for the service.

Ms. Rosemary Degoh, FNP is our dedicated adult nurse practitioner. She values providing patients with high value comprehensive care.

Ms. Degoh has over 5 years experience and specializes in preventive care, primary care, chronic disease management and telehealth urgent care. She works closely with Dr. Hayes to ensure patients have a solid comprehensive plan.

What is Concierge Medicine or "Membership Medicine"?

Concierge Medicine is a membership service in which clients pay for additional care services provided by an exclusive personal care physician.

Which insurance plans do you accept?

We accept Anthem, Blue Cross Blue Shield, Cigna, Aetna, Humana, United HealthCare, Tricare, Medicare and fee for service(credit card, HSA/FSA card and cash). Sorry, we do not accept personal checks. Deductibles, co-insurance and copays are expected to be paid at the time of service.

Do you treat children?

No. Dr. Hayes is an internist which is an adult care doctor. If you desire concierge services for your children, Dr. Hayes can refer you to a concierge pediatrician.

Are you accepting new patients?

Yes! Dr. Hayes is accepting new patients!
